Hyaluronidase is an enzyme that breaks down hyaluronic acid. Your body already produces it; the injectable form is simply concentrated. Placed into an area of filler, it cleaves the gel apart within hours and your body clears the fragments over the following days.
This is the reason hyaluronic acid dominates the filler market. Almost nothing else in aesthetics is genuinely reversible. If a result is wrong — too much, wrong plane, migrated, asymmetric, or just not what you pictured — it does not have to be lived with for a year.
The important caveat is that it only works on hyaluronic acid. Sculptra, Radiesse, permanent fillers such as PMMA, silicone and fat transfer do not respond to it at all. Working out what you actually have is the first thing we do, and if it is not hyaluronic acid we will tell you honestly rather than inject you and take the fee.

We work out what is actually there
Which product, roughly when, how much and in what plane. If you do not know, we assess by feel and appearance. This is also where we determine whether it is hyaluronic acid at all — because if it is not, hyaluronidase will not touch it.

Allergy is considered
Hyaluronidase is derived from several sources and allergic reaction, while uncommon, is possible. We take a full allergy history and can patch test beforehand where there is any reason for concern.

Injected directly into the areas holding product. Dosing is judged by volume and how long the filler has been in place — older, more integrated filler generally needs more.

This is the single most important thing on this page. Hyaluronidase breaks down hyaluronic acid and nothing else. Sculptra is poly-L-lactic acid and does not respond. Radiesse is calcium hydroxylapatite and does not respond. Bellafill and other PMMA fillers are permanent and cannot be dissolved by anything. Silicone and fat transfer are likewise not removable this way. If you do not know what you had, we will assess by feel, appearance and timeline — and if we believe it is not hyaluronic acid we will tell you at consultation rather than inject you, take the fee and let you discover it a fortnight later.

Two things happen at once. Hyaluronidase temporarily breaks down some of your own natural hyaluronic acid alongside the filler — that recovers within a day or two. More significantly, tissue that has been stretched by filler for months or years needs time to retract, and in the interim the area can look flat, hollow or lax. This is temporary and it is normal, but it catches people out badly if nobody warns them. It is also the reason we will not let you rush into a rebuild: injecting into tissue that has not yet returned to baseline is how the original problem repeats itself.

Hyaluronidase is not only a correction tool. It is the emergency treatment for vascular occlusion — filler entering or compressing a blood vessel — which is the rare but genuinely serious complication of any filler treatment. Managing it successfully depends on recognising it immediately and having hyaluronidase available right then, not ordering it in. We keep it on site at every appointment, which is a reasonable question to ask any clinic before they inject you. If you are currently experiencing blanching, escalating pain, dusky skin or a vision change after filler anywhere, contact your injector or an emergency department now rather than reading further.

A large share of the dissolving we do is on filler placed at other clinics, and patients frequently arrive braced for a lecture. There will not be one. You do not need paperwork, a product name, a receipt or the name of the person who treated you — helpful if you have it, entirely unnecessary if you do not. We will assess what is there, tell you what we think it is, and be straight about whether we can help. Plenty of clinics decline to touch work they did not place, which leaves people stuck with a result they dislike and nowhere obvious to go. That is not a position we are willing to put anyone in.

Will hyaluronidase work on what you have?
This is worth checking before you book anywhere. Hyaluronidase dissolves hyaluronic acid and nothing else. If you had Sculptra, Radiesse or a permanent filler, no amount of it will help — and a clinic that injects you without establishing which you have is not doing its job.

Katie Manhardt is a board-certified family nurse practitioner and the founder of KORA Aesthetics, and a national clinical trainer for Galderma and Prollenium — teaching injection technique and facial anatomy to other medical professionals across the country.
Correcting other injectors' work is a significant and deliberate part of the practice. Her position is that nobody should be left living with a result they dislike because the clinic that placed it will not remove it. Every appointment at KORA is performed by Katie or by Victoria Patterson, NP.

How to prep & what to expect after
Helpful, but not required
- Bring any product names, dates or records if you have them. If you have none, come anyway — we assess by feel, appearance and timeline.
- Tell us about any allergies, particularly to bee stings, or any previous reaction to hyaluronidase. We can patch test where there is reason for concern.
- If medically appropriate and cleared by your physician, stop blood thinners, NSAIDs, fish oil and vitamin E for 48 hours.
- Allow at least three weeks before any event. The area often looks flatter than expected for the first week or two.
- Tell us if you have a cold sore history and lips are being treated — we may prescribe antiviral cover.
- Reschedule if you have an active infection in the area.
- Let us know if you are pregnant, trying to conceive or breastfeeding.
The first week
- Expect immediate swelling from the injection itself. This is not the filler and it is not a failure — it settles within a few days.
- Ice intermittently for the first 24 hours and sleep with your head elevated for two nights.
- Most of the dissolving happens in the first 24 to 48 hours. The difference is usually obvious by the second morning.
- The area may look flat or hollow for a week or two while stretched tissue retracts. This is temporary and expected.
- No strenuous exercise, sauna or hot tub for 48 hours.
- Avoid alcohol for 24 hours.
- Do not book a rebuild for at least two to four weeks. Injecting into tissue that has not returned to baseline repeats the original problem.
- Call us for spreading redness, heat, hives, wheezing, swelling of the tongue or throat, or any difficulty breathing. Allergic reaction to hyaluronidase is uncommon but needs immediate attention.
