What PRF is

Your own biology,

Platelet-rich fibrin is made from a small sample of your own blood. We draw it much as any clinic draws routine bloodwork, spin it in a centrifuge for around ten minutes, and separate the layer rich in platelets, white blood cells and fibrin. That layer is injected back into the area being treated.

Platelets are the body's repair signal. Concentrated and placed where you want regeneration to happen, they recruit the cells that build collagen, improve microcirculation and — in the scalp — extend the growth phase of a hair follicle.

The distinction from PRP is the spin. PRP is centrifuged fast with an anticoagulant added, producing a liquid that releases its growth factors quickly, largely within a day. PRF is spun slowly in a plain tube with nothing added, which allows fibrin to form a natural mesh. That mesh holds the platelets in place and releases growth factors gradually over seven to ten days — and it retains stem cells that PRP's processing discards.

How PRF works

The whole process happens in one appointment, and most of it takes place while your numbing cream works.

A small draw

Much like routine bloodwork and over in under a minute. The amount depends on the area being treated — under-eyes need considerably less than a full scalp.

Slow spin, plain tube

Around ten minutes in the centrifuge at low speed, with no anticoagulant added. This is the step that distinguishes PRF from PRP and it is why the fibrin is able to form at all.

A fibrin scaffold forms

Fibrin creates a natural mesh that physically traps platelets, white blood cells and stem cells. Without an anticoagulant fighting it, the body's own clotting architecture assembles in the tube.

Placement

Cannula under the eyes, where blunt technique markedly reduces vascular risk. Fine needle across the scalp in a grid. The plane matters as much here as with any injectable.

Sustained release, then regeneration

Growth factors release from the scaffold over seven to ten days, recruiting fibroblasts and improving microcirculation. New collagen builds over the following weeks, which is why results appear at four to eight rather than immediately.

PRF under the eyes causes firm swelling for two to four days, more pronounced initially than hyaluronic acid filler. Results build gradually from around four weeks and reach full effect at eight to twelve weeks. PRF typically lasts 12 to 18 months.

PRF under the eyes

The swelling is firm,
and it surprises people

Under-eye PRF swells more in the first few days than hyaluronic acid filler does, and the swelling is firm rather than soft — patients frequently describe it as feeling like a small pad under each eye. It peaks at 24 to 72 hours and settles by day four to seven. This is not a complication; it is the fibrin matrix in place before the tissue absorbs it. Knowing that in advance is the difference between a patient who waits calmly and one who panics on day two. Plan around it: treat on a Thursday, allow three weeks before anything photographed, and expect no visible result at all for the first month.

The trade-off
against filler

Hyaluronic acid filler under the eyes can be dissolved within days if you dislike it. PRF cannot — there is no hyaluronidase equivalent, because it is your own tissue. That is a genuine consideration and it cuts both ways. PRF carries no risk of the blue-grey Tyndall tint that superficial filler can cause here, no risk of the water-attracting puffiness that tear trough filler sometimes develops, and no allergy risk whatsoever. But it is also not reversible, so we treat conservatively and build across sessions rather than trying to achieve everything at once.

Hair restoration

It works on follicles
that are thinning, not gone

PRF improves the blood supply to a follicle and extends its growth phase, which is genuinely useful where hair is miniaturising — becoming finer, shorter and lighter. It does nothing at all where the follicle has been lost entirely, because there is nothing left to stimulate. This makes timing the most important variable in the whole treatment: patients who come at the first sign of thinning do considerably better than those who wait years. We will assess honestly and tell you if we think you are past the point where PRF will help, and where a surgical referral or medical therapy is the better route.

Why the spin speed

PRP and PRF are used almost interchangeably in marketing and they are not the same treatment. PRP is centrifuged at high speed with an anticoagulant added to keep it liquid, producing a solution that releases its growth factors quickly — the majority within the first twenty-four hours. PRF is spun slowly in a plain tube with nothing added at all. Without an anticoagulant, fibrin forms a natural mesh that traps platelets, white blood cells and stem cells, releasing growth factors gradually across seven to ten days. For skin regeneration and for hair, where the follicle cycle runs over weeks rather than hours, we consider sustained release the better mechanism — and the absence of any additive means what goes back in is purely your own blood.

PRF or PRP, and when is filler better?

For under-eyes the real decision is between PRF and hyaluronic acid filler, and it turns on one question: is your hollowing a volume problem or a skin-quality problem? Filler restores volume immediately and can be dissolved. PRF improves skin thickness and circulation gradually and cannot. PRP is the same idea as PRF processed differently and less well. EZ Gel sits between them.

Under-eyes are the most technically demanding area on the face. If a practice offers only one of these, that is worth knowing.

How to prep & what the first week looks like

Hydration matters more than you would think

  • Drink plenty of water the day before and the morning of. It makes the draw easier and genuinely improves the PRF yield.
  • Eat a proper meal beforehand. Injections on an empty stomach after a blood draw are the most common cause of feeling faint.
  • Avoid alcohol for 24 hours and caffeine on the morning of treatment.
  • Tell us about any blood thinners, platelet disorders, anaemia or recent illness — these affect both the draw and the result.
  • If treating under the eyes, allow at least three weeks before any event. Firm swelling lasts two to four days.
  • Reschedule if you have an active infection or are unwell.
  • Let us know if you are pregnant, trying to conceive or breastfeeding — we will not treat you.

Who should not have PRF?

Anyone with a platelet or bleeding disorder, anyone on anticoagulants without physician clearance, anyone with an active infection, anyone with certain blood-borne conditions or active cancers, and anyone pregnant or breastfeeding.
