Microneedling is collagen induction therapy. A sterile cartridge of very fine needles creates thousands of microscopic channels in the skin. None of them is a wound in any meaningful sense, but together they are enough to trigger the body's repair cascade — and repair means new collagen and elastin laid down in the layer where texture and scarring actually live.

The result is not a glow that fades by the weekend. It is structural remodelling that builds over eight to twelve weeks and keeps improving between sessions, which is why we treat in a course of three or four rather than selling a single appointment.

Adding PRF changes the arithmetic. We draw a small amount of your blood, spin it, and apply the platelet-rich fibrin into the channels the needles just made. Those channels close within about an hour, so the growth factors go where topical serums cannot. Nothing synthetic is involved — it is your own biology, concentrated.

What happens
in the skin

The needles are not the treatment. The healing response they provoke is the treatment. Here is the sequence.

Micro-channels are created

SkinPen's sterile cartridge oscillates vertically to create thousands of microscopic channels at a set depth. Vertical motion matters — rollers drag laterally and tear rather than puncture.

The wound-healing cascade begins

The body reads the channels as injury and floods the area with platelets and growth factors. This is the inflammatory phase, and it is why the skin flushes for a day or two.

PRF goes in while the channels are open

The channels close within roughly an hour. Applying platelet-rich fibrin during that window delivers concentrated growth factors into the dermis rather than onto it, which no topical serum can do.

New collagen is laid down

Fibroblasts produce fresh collagen and elastin over the following weeks. The old, disorganised collagen that forms a scar is gradually replaced with better-organised tissue.

Remodelling continues for months

Collagen keeps maturing for eight to twelve weeks after each session. This is why results build between appointments and why we assess the course at the end rather than after one visit.

Skin assessment

We look at scar type, texture, pore size and pigment, and set the depth per zone. This is also where we tell you if microneedling is the wrong treatment for what you have.

Blood draw and spin

A small draw, much like routine bloodwork, then eight to ten minutes in the centrifuge at low speed. No anticoagulant is added, which is what makes it fibrin rather than plasma.

Numbing

Topical anaesthetic for 20 to 30 minutes. This is the longest part of the appointment and the reason most patients describe the treatment as comfortable.

Microneedling with PRF

Fifteen to twenty minutes of passes across the face, depth adjusted by zone, with PRF applied into the open channels as we work.

Aftercare

You leave flushed, like moderate sunburn. We send you home with written aftercare and the PRF left on the skin — nothing is washed off for the rest of the day.

Microneedling causes 24 to 48 hours of redness resembling mild sunburn. Texture improves from around week two and collagen remodelling continues for 8 to 12 weeks. Most patients need 3 to 4 sessions spaced 4 to 6 weeks apart.

PRF under the eyes

The area patients
ask about most

PRF injected under the eyes is a different treatment from microneedling, and the two are constantly confused. Microneedling with PRF improves crepey texture on the surface. PRF injected into the tear trough addresses hollowing and skin quality in the deeper tissue, and typically takes two to three sessions spaced four to six weeks apart. Expect firm swelling for two to four days — PRF swells more than filler initially and settles into a softer, more gradual result. It usually lasts twelve to eighteen months and, because it is entirely your own blood, it cannot be dissolved. That is worth understanding before you choose it over hyaluronic acid filler.

Who it will not suit

When we say
no

Microneedling is not appropriate during active inflammatory or cystic acne — puncturing inflamed skin spreads bacteria and worsens it. We also decline within six months of isotretinoin, with a history of keloid scarring, during pregnancy or breastfeeding, over active cold sores or infection, and where the true concern is pigment rather than texture. In several of those cases a VI Peel or a course of medical-grade skincare is the better starting point, and we would rather redirect you than take the booking.

PRF EZ Gel

Your own plasma,
as a filler

EZ Gel is PRF heated and then cooled, which denatures the albumin into a gel with the consistency of a soft hyaluronic acid filler — except it is entirely your own blood. It suits patients who want volume restoration under the eyes or in fine lines but would rather not have synthetic product placed. It gives less lift than HA filler and lasts around nine to twelve months, and it is a genuinely different decision rather than a straight substitute.

PRF vs PRP

Why we use PRF
and not PRP

Both are concentrated from your own blood, and the two are used almost interchangeably in marketing. They are not the same. PRP is spun fast with an anticoagulant added, which yields a liquid that releases its growth factors quickly — largely within the first day. PRF is spun slower with no additives at all, which allows a natural fibrin scaffold to form. That scaffold traps platelets and white blood cells and releases growth factors gradually over seven to ten days, and it also holds stem cells that PRP's processing discards. For skin and hair we consider the slower, sustained release the better mechanism, and the absence of any additive means the treatment is entirely autologous.

Which skin treatment do you actually need?

These four get grouped together as skin treatments and they do different things. Microneedling rebuilds collagen for texture and scarring. PRF injections treat under-eye hollowing and hair thinning. EZ Gel restores volume using your own blood. A VI Peel targets pigment. If your concern is brown patches rather than texture, no amount of microneedling will fix it.

How to prep & what to do after

Start about one week ahead

  • Stop retinoids, glycolic and salicylic acids and any exfoliating actives for 5 to 7 days beforehand.
  • Avoid sun exposure and self-tan for two weeks. We cannot treat sunburnt or freshly tanned skin.
  • No waxing, threading or hair removal on the area for a week.
  • Hydrate well the day before and morning of — it makes the blood draw easier and the PRF yield better.
  • Tell us about any cold sore history. Microneedling can trigger an outbreak and we may prescribe antiviral cover.
  • Reschedule if you have active inflammatory acne, an infection, or open skin in the area.
  • Arrive without makeup where possible, and eat beforehand.

How long is the downtime?

24 to 48 hours of redness resembling mild sunburn. Many patients work from home the next day and are comfortable in mineral makeup by day two. Some dryness or light flaking follows on days three to five.

PRF

What is PRF and why do you use it instead of PRP?

PRF is platelet-rich fibrin, concentrated from your own blood at a lower spin speed with no anticoagulant added. That allows a fibrin scaffold to form, which releases growth factors gradually over 7 to 10 days and retains stem cells that PRP processing discards. PRP releases faster, largely within a day.

How long does PRF under the eyes last?

Typically 12 to 18 months. Most patients need two to three sessions spaced four to six weeks apart to reach the full result, and it builds gradually rather than appearing immediately.

What are the side effects of PRF under the eyes?

Firm swelling for two to four days is normal and more pronounced than with hyaluronic acid filler, along with possible bruising and tenderness. Because PRF is entirely your own blood there is no allergy risk — but equally it cannot be dissolved, which is worth weighing before choosing it over filler.

Safety

Is microneedling safe for darker skin tones?

Yes. Because it does not use heat or light, microneedling carries a much lower risk of post-inflammatory pigmentation than many resurfacing lasers, and is safe across all Fitzpatrick types. Depth still needs to be set appropriately.

Who should not have microneedling?

Anyone with active inflammatory or cystic acne, anyone within six months of isotretinoin, anyone with a history of keloid scarring, anyone pregnant or breastfeeding, and anyone with an active infection or cold sore in the area. We screen for all of this.

Can I combine microneedling with Botox or filler?

Yes, though usually not on the same day. We normally schedule microneedling two weeks after injectables so swelling has resolved and the treatments are assessed independently.
